Check the ItemType that the form is associated with. My guess is that the data source for one of the properties did not get imported with the package. Usually a List referenced by a propety.
I new version of the Solutions Upgrade Tool will be available with Aras Innovator 8.2 that has increased reference checking for this situation. So that you will know about the problem earlier.
