I want to add a form on the relationship's tab.

Hi Leena,
Find the Relationship Type (Under Administration) for the Tab that you want to change the form for.
Edit the Relationship Type item.
On the "Relationship Views" tab add a new relationship and specify either the "Start Page" which is an external Html page or a new "Form" which is an internal Innovator form.
Save.
Done.
